The purpose of this form is to allow students "easy" access to me. Please be sure that you complete all the relevant information, making sure to provide your e-mail address and phone number. Once I receive your request for an appointment, I will check my calendar and send you either a confirmation of the appointment date and time or will inform you if there are conflicts. You should receive a response from me within 36 - 48 hours. If you do not hear from me, please call or send the form again!

Reason for the appointment

This doesn't have to be detailed or specific, just provide a general idea of what you might need and how the instructor might be of some assistance to you.

You should receive verification of your appointment request within 48 hours. Because of the two-day turn around, be sure that your request for appointment is within reason. In other words, do not schedule or make a request for an appointment for an appointment on the same day!!
