5 questions

1. WHO (Always the patient!)

2. will do WHAT (something they desire to do or accomplish, not "PT-speak")

3. under what CONDITIONS

• environmental variables: "across uneven grassy surfaces" or "down 5 steps"
• patient variables: "touch-down weight bearing" or "using a power wheelchair."
• incorporate specific elements of a measurement into the goal, eg, time to perform an activity, exertion scales, etc.

4. HOW WELL

• amount of assistance
• number of successful attempts before goal achieved
